We were happy to present Grant funds to Health & Welfare Council of Long Island. Our funds will support HWCLI staff providing direct assistance to clients in education, prescreening and application assistance for SNAP enrollment. This will be in partnership with Catholic Health Systems discharge planning team to create a referral system for food insecure patients.
